Our Supported Living

Everyone deserves a home to feel proud of. But our supported living is not just about modern, homely surroundings. It is about providing support that fits seamlessly around you, with uniquely rewarding relationships that will help to build your confidence and independence.

We represent a gap in the pathway to full independence and results in people remaining in 24-hour residential care longer than they need to.

Our support

We offer different types of living environments, including independent flats and shared homes with communal spaces. Our team will support you to understand your options, and help you to find the perfect home.

We support male or female adults over 18 years of age with one or a combination of conditions, which may include;

· Mental health needs / dual diagnosis

· Physical and learning disabilities

· MOJ Patient

· Challenging behaviour

· Autism

We offer both core and 1:1 support, all of which is designed around the individual needs and wishes of each person by our dedicated and specialist teams.

We weave quality into everything we do. Our teams will work closely with you to co-create a personalised support plan focusing on your goals, interests, skills, and strength.

Functions of our supported living accommodation

· A wide range of safe and homely living spaces, including self-contained apartments and shared living.

· Support with developing daily living skills, independence and accessing the community

· Unique care pathways providing step-up and step-down services

· Staff availability 24 hours per day

· Integration of assistive technology within properties to maximise independence

· May have a history of failing to engage with services
